These Terms and Conditions ("Terms") govern the use of services provided by Alderson Family Connection P.C ("Practice," "we," "us," or "our"), a professional corporation, including clinical counseling services and the use of our on-site salt therapy (halotherapy) room. By scheduling an appointment, receiving services, or entering our premises, you agree to these Terms.

1. Professional Services

A. Clinical Counseling Services

Clinical services are provided by licensed professionals in accordance with Illinois law, HIPAA, and the ethical standards of the social work profession. Counseling services are not a guarantee of specific outcomes, and progress varies by individual.

B. Salt Therapy (Halo-therapy) Services

The salt therapy room is offered as a complementary wellness service.

• Salt therapy is not medical treatment, psychotherapy, or a substitute for medical care.
• No diagnosis, treatment, or cure of medical conditions is claimed or implied.
• Clients are responsible for consulting with their medical provider regarding any health concerns prior to use.

2. Eligibility and Use of Services

• Clients must complete all required intake, consent, and screening forms prior to receiving services.
• Minors may receive services only with appropriate parental or legal guardian consent.
• We reserve the right to refuse or discontinue services if participation is deemed unsafe, inappropriate, or noncompliant with these Terms.

3. Appointments, Cancellations, and No-Shows

• Appointments must be cancelled or rescheduled at least 24 hours in advance.
• Late cancellations or no-shows may result in a fee of [Insert Fee].
• Repeated no-shows may result in termination of services.

4. Fees, Payments, and Insurance

• Payment is due at the time of service unless prior arrangements are made.
• We may accept private pay, insurance, or other approved payment methods.
• Clients are responsible for understanding their insurance benefits and any out-of-pocket costs.
• Salt therapy services may not be covered by insurance and are typically self-pay.

5. Salt Therapy Room Rules and Safety

To ensure safety and comfort:

• Follow all posted guidelines and staff instructions.
• Disclose relevant medical conditions prior to use.
• Food, drinks, smoking, and electronic recording devices are prohibited in the salt room.
• Use is at the client’s own risk.
• Failure to comply may result in removal from the session without refund.

6. Confidentiality and Privacy

• All clinical services are confidential and protected under HIPAA and Illinois law.
• Wellness-related information collected for salt therapy is handled in accordance with our Privacy Policy.
• Exceptions to confidentiality apply as required by law (e.g., risk of harm, abuse reporting, court orders).

7. Electronic Communication

Electronic communications (email, text, telehealth platforms) may involve privacy risks. By opting into electronic communication, you acknowledge and accept these risks. You may opt out at any time.

8. Telehealth Services 

• Telehealth services are provided in compliance with Illinois law.
• Clients are responsible for ensuring a private and secure environment during sessions.
• Technical disruptions may occur and are not grounds for refunds.

9. Conduct and Respectful Use of Premises

• Clients and visitors are expected to behave respectfully toward staff and others.
• Harassment, disruptive behavior, or unsafe conduct will not be tolerated.
• We reserve the right to terminate services or ask individuals to leave the premises.

10. Limitation of Liability

To the fullest extent permitted by law:

• The Practice is not liable for adverse reactions or outcomes related to salt therapy use.
• We are not responsible for personal belongings lost or damaged on the premises.
• Liability for clinical services is limited to the scope permitted under applicable professional standards and law.

11. Indemnification

Clients agree to indemnify and hold harmless Alderson Family Connections P.C., its owners, employees, and contractors from claims arising out of misuse of services, failure to disclose relevant health information, or violation of these Terms.

12. Intellectual Property

All materials provided by the Practice (forms, handouts, website content) are the intellectual property of the Practice and may not be reproduced without written permission.

13. Changes to Terms and Conditions

We reserve the right to modify these Terms at any time. Updated Terms will be posted in the office and/or on our website. Continued use of services constitutes acceptance of revised Terms.

14. Governing Law

These Terms are governed by the laws of the State of Illinois.
